POSITION SUMMARY:
Successfully promote and support the mission of Catholic Charities of Northeast Kansas as guided by the Catholic Social Teachings by providing supportive housing services. Each client is assigned a case manager whose goal it is to assist with gaining more independence. Case Managers work directly with clients to establish a treatment plan according to their needs and interest. Within this treatment plan, clients will receive support from case manager to obtain employment, income, educational courses, maintain appointments, and continuum of care with local resources. Clients are required to have weekly visits with assigned case manager. Catholic Charities of Northeast Kansas is an equal opportunity employer. ESSENTIAL FUNCTIONS:
- Accomplishes clients’ care by assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress; facilitating interdisciplinary approaches; monitoring staff performance.
- Admits new clients by reviewing records and applications; conducting orientations.
- Determines clients’ requirements by completing intake interviews; determining need for therapeutic medical, psychosocial, psychiatric evaluations, and physical health; reviewing evaluations, treatment objectives, and plans.
- Establishes treatment programs by setting schedules and routines; coordinating services provided; arranging resources, including transportation and escort.
- Monitors cases by verifying clients’ attendance; observing and evaluating treatments and responses; advocating for needed services and entitlements; obtaining additional resources; intervening in crises; providing personal support.
- Maintains clients’ records by reviewing case notes; logging events and progress.
- Communicates clients’ progress by conducting weekly interdisciplinary meetings and evaluations; disseminating results and obstacles to therapeutic team and family; identifying treatment influences.
- Prepares clients’ discharge by reviewing and amplifying discharge plans; coordinating discharge and post-discharge requirements; providing resources.
- Attend agency staff meetings, TH/RRH meetings, trainings and other meetings as specified
- Performs other duties as assigned
MINIMUM POSITION REQUIREMENTS:
- Demonstrate a passion for the mission of Catholic Charities
- Must be willing to perform by the agency’s values: love, patience, kindness, faithfulness, self-control and joy
- Catholic Charities promotes a diverse workforce. - Must complete VIRTUS training, background screening, drug testing, and any ongoing training as required
- 2 years of experience in a Social Service related field preferred
- Ability to work effectively in an environment that is very diverse
- Must possess a valid driver’s license, dependable transportation, current vehicle insurance and a verified safe driving record.
PHYSICAL REQUIREMENTS:
- Prolonged periods of sitting at a desk and working on a computer, standing, bending and moving
- Must have ability to effectively communicate with others
- Must be able to lift up to 10 pounds
- May be exposed to infectious diseases
- Must have the physical ability and appropriate licensure for driving
