Job Title:  Community Aging Specialist
 
Position Summary
The Community Aging Specialist supports older adults in maintaining independence and safely aging in place through holistic assessment, resource connection, and individualized support. Working within a mission-driven nonprofit organization, this role empowers adults aged 60+ to remain in their homes and communities for as long as it is safe to do so.  This position focuses on building trusting relationships, supporting client-centered goals, and collaborating with community partners to reduce barriers and enhance quality of life.

Key Responsibilities
Advocacy, Assessment & Individualized Support

  • Conduct person-centered assessments to identify strengths, needs, safety considerations, and personal goals
  • Develop individualized support plans that promote independence and well-being
  • Connect older adults to essential community resources
  • Assist clients with completing applications and accessing services
  • Build and maintain partnerships with local agencies, senior service providers, and volunteer networks
  • Offer education on fall prevention, home safety, social isolation, and other aging-related concerns
Community Outreach & Education
  • Participate in outreach events, presentations, and community engagement initiatives.
  • Serve as the organization’s knowledgeable and professional representative for aging adult service
Documentation & Compliance
  • Maintain accurate and timely documentation, including case notes and follow-ups
  • Ensure client confidentiality in accordance with ethical standards and regulations
 
Qualifications
  • Bachelor’s degree in Social Work, Gerontology, Human Services, Psychology, or a related field (or equivalent experience)
  • Experience working with older adults, vulnerable populations, or in community-based settings
  • Knowledge of aging-related issues and resources that support independence and safety.
  • Strong interpersonal, communication, and problem-solving skills
  • Ability to collaborate effectively with diverse individuals and service providers.
 
Preferred Skills & Experience
  • Experience in nonprofit or community-based organizations with the ability to serve a diverse aging population
  • Familiarity with public benefits programs such as Medicare, Medicaid, Social Security, and utility/food assistance

Job Type: Part Time

Salary Range: $30,000 - 49,999
