Transition Specialist

GENERAL DESCRIPTION
The Transition Specialist will support consumers in transitioning from skilled nursing facilities back into the community, empowering them to live as independently as possible.
ESSENTIAL JOB FUNCTIONS
- Communicate with MFP/Show-Me Home Social Services personnel and nursing home social workers to identify appropriate candidates for transition services.
- Assist consumers in securing appropriate housing.
- Coordinate transition plans between consumers and skilled nursing facilities.
- Provide follow-up support to ensure successful community reintegration and independent living.
- Maintain accurate documentation of consumer interactions and complete administrative tasks as assigned.
- Advocate effectively on behalf of consumers.
- Maintain regular and reliable attendance.
REQUIRED SKILLS
Associate’s degree in Social work, Human Services, Rehabilitation Counseling, Psychology, Sociology, or a related field; or a high school diploma/GED with 2–3 years of relevant experience in:
- Social services
- Disability support
- Community outreach
- Housing assistance
- Healthcare or long-term care coordination
Valid Driver’s License
PREFERRED SKILLS
- Bachelor’s degree in social work, Human Services, Rehabilitation Counseling, Psychology, Sociology, or a related field.
- Experience working with individuals transitioning from institutional settings to community-based living.
- Familiarity with Medicaid waivers, housing programs, and community resources.
- Knowledge of disability rights, independent living philosophy, and person-centered planning.
- Strong documentation and case management skills.
- Proficiency in Microsoft Office and case management software
- Bilingual or multilingual abilities are a plus

WORK ENVIRONMENT & PHYSICAL DEMANDS
This position requires frequent travel throughout the Kansas City Metropolitan area to meet with consumers, service providers, and community partners. The role may involve physically demanding tasks such as kneeling, lifting up to 25 pounds, and assisting with mobility equipment or home setup. Work is performed in a variety of environments, including nursing facilities, private residences, and community settings. Flexibility and the ability to adapt to different physical and social environments are essential.
