GENERAL DESCRIPTION The Consumer Directed Services (CDS) Home Visit Specialist is responsible for facilitating all annual home visits for each Consumer on the CDS program at The Whole Person. The main responsibility of the CDS Case Manager is to conduct the annual visits in order to evaluate the actual living conditions of the Consumer’s home, assessing for any safety concerns and ensuring quality of care as described in the Consumer’s plan of care.
