Coordinated Entry Housing Navigator

Metro Lutheran Ministry (MLM), a nonprofit social services provider championing innovative solutions to food and housing insecurity, seeks to hire a full-time Coordinated Entry Housing Navigator. This position will support HUD related Coordinated Entry System services in the Northland, in partnership with the Missouri Balance of State MO-606 Continuum of Care. 

Work Schedule:                 Monday-Friday, 8:30am - 5:00pm; some weekends or evenings as needed

Job Description:                 The Coordinated Entry Housing Navigator (CEHN) plays a central role in the Missouri Balance of State Coordinated Entry System (CES), serving Region 4 of the MO BoS, working with households experiencing homelessness who are seeking permanent housing. The CEHN serves as the primary point of entry into the local CES, working with unhoused households to schedule and complete CES assessments by phone or in person, providing diversion and triage resource referrals, and completing ongoing outreach to ensure accurate update of CES client data. The CEHN also performs landlord and housing provider outreach, engagement, and recruitment to support positive diversion and permanent housing outcomes.

Education:            Bachelors in Social Work, Sociology, or Psychology preferred but not required
 
Experience:          Two years prior experience in a similar role is preferred. Current KS/MO driver’s license and proof of insurance (must submit to MVR, MO FCSR, and criminal background check).
 
Duties and Responsibilities:
 
The CEHN shall possess excellent communication skills and demonstrate patience and kindness while working under pressure and delivering quality service, upholding the values and integrity of MLM.
 
The CEHN will support homeless households, utilizing a strengths-based model, by performing the following: 

  • Conducts CES common assessment tool and triages households experiencing homelessness through diversion and resource referrals
  • Performs outreach to participants of the CES to update current living situation and provide additional diversion and referral resources
  • Identifies appropriate diversion and referral interventions to help resolve housing crises
  • Assists households with collecting documents necessary for permanent housing
  • Assists households with accessing permanent housing opportunities including Permanent Supportive Housing, Rapid re-Housing, subsidized housing, and affordable housing, through diversion and referral resources
  • Connects households to supportive services, as needed, including behavioral health, substance use, employment, mainstream benefits, etc.
  • Manages and tracks participant information in the Homeless Information Management System (HMIS), WellSky
  • Inputs & reviews client data in HMIS ensuring accuracy, completeness, and timeliness
  • Attends phone/digital case conferencing and regional meetings to coordinate services, housing advocacy and updates with other nonprofit providers in Region 4
  • Performs outreach and engagement with Northland landlords and housing providers to encourage acceptance of HUD funded housing program funding
  • Maintains and updates a landlord and housing provider database/directory
  • Works collaboratively to assist households in receiving equitable access to CES as well as to diversion and referral resources
  • Other duties as assigned
Compensation:         MLM offers a competitive compensation package for qualified candidates.
